Filing Requirements
1. Details of Applicants i.e. full name, address and nationality, or details of legal entity;
2. Details of Authors i.e. full name, address and nationality (the applicant is not the author);
3. Type of Work;
4. Title of Work;
5. Sample of Work or its substitute;
6. Description of Work;
7. Date and Place of First Publication of the Work;
8. A simply signed Special Power of Attorney;
9. A simply signed Assignmentfrom the Author(s) to the Applicant (the applicant is not the author);
10. Copy of the Applicant’s ID (KTP)*;
11. Official excerpt of Deed of Establishment or its notarized copy (the applicant is a legal entity)*;
12. Copy of the Applicant’s tax registration number/NPWP (the applicant is a legal entity)*.
